Japanese Phrases for work

I work in an airport giving massage and I get a Japanese person who speakes no English about once a month and there are a few things that I just don't know how to ask them?

We have a medical waver that we need them to sign and we also have people print their name.
How would I ask them to read the medical waver? We have it written in Japanese .

How do you ask them to Sign and print their name?

Also how would I ask if they would like a 15min or 30min massage? (What is the word for massage.)

Also how would you ask if they would like a foot massage?

Any help would be a great help.

Massage = massagy - very easy
15 min - ju go fun
30 min - san juppun
name - o nammae - Japanese people will normally print when asked to write their name
alphabet please (i.e. not kanji) - alphabeto kudasai
sign - signu
foot - ashi

I have spelt them more phonetically that how they are really spelt

Medical waivers etc a bit too technical for me

do you know how I would ask someone to please read this in polite form. I can just point to the waiver for them to read but it would be nice to be able to ask them to.

I will look up about the massage....just to be 100%. I know the word for massage....but I don't want to give an incorrect sentence.
