<Help> Making a reservation by phone

Hi there,

I am planning to do strawberry picking in kishikawa in April 2012 (http://www.kanko-kinokawa.jp/member/7212/).

However, I am required to make a reservation by phone. Can you provide some sentences that I can use to make the reservation?

P.S. I can speak simple Japanese but I am not able to construct sentences for this scenario.
